I voluntarily and of my own free will, elect to allow my child and or children to participate in the USAYESS Youth Education in Shooting Program. Furthermore, I acknowledge and am aware that there are certain risks and hazards involved in the above-named program that may result in injury or death to my child or children or other child/ children athletes including, but not limited to those hazards associated with this sport, playing conditions, equipment, and other participants. I hereby release, discharge, and agree not to sue USAYESS, or the organization's owners, officers, agents, servants, associations, teams, affiliates, employees, volunteers or any person or entity connected with USAYESS for any claim, damages, costs, or cause of action which I have or may in the future have as a result of injuries or damages sustained or incurred by the above-named minor from whatever cause including but not limited to the negligence, breach of contract or wrongful conduct of the parties hereby released. Participant Waiver, Release of Liability & Indemnification Agreement I, the undersigned participant, acknowledge, agree, and understand that: Voluntarily and of my own free will, elect to participate in USAYESS Youth Shooting Program. Furthermore, I agree that I am in good health and proper physical condition to participate in youth shooting sports. I understand that there are certain risks and hazards involved in participating in youth shooting sports that may result in injury or death to me or other athletes including, but not limited to those hazards associated with, playing conditions, equipment, and other participants. I understand that youth shooting sports are dangerous to me and or other athletes and may result in serious injury or death. I understand that the very nature of youth shooting sports is hazardous and risky, including, but not limited to, the acts of shooting, walking, running, jumping, stretching, and collisions with other athletes and with stationary objects, all of which can cause serious injury or death to me and to other athletes. Further, I, the undersigned athlete, agree that in consideration for the right to participate in youth shooting sports and in consideration for permission to participate: I voluntarily elect to accept and assume all risks of injury incurred or suffered by me (a) while practicing or playing as a member of the team so designated, (b) while serving in a non-playing capacity as a team member during practice or play by other teams or by other athletes on my team, and (c) while on or upon the premises of any and all facilities arranged for by my team or league for practice or play. I understand that my behavior and conversation reflect on the team and the program, and that unsportsmanlike behavior on my part may result in me being asked to leave the area. I understand that negative and divisive behavior on my part could also result in my child being disqualified or even removed from USAYESS. USAYESS provides events that showcase the safety of equipment, and outdoor activities in a fun-filled atmosphere. USAYESS will ensure the transformation and educational potential of student-athletes with opportunities to practice and compete on a level playing field, and we are committed that those athletic opportunities involve new skills, developing individual goals, planning, potential, and fostering an appreciation of collaboration and teamwork in the competitive arena of athletics. Our member athletes are students first, and foremost. The well-being of our student-athletes is first and foremost. USAYESS strives to develop programs and events that encourage boys and girls to develop camaraderie, practice habits of mind and body that encourage healthy living, and learn valuable life lessons. Every member will strive for excellence in all programs and events; however, we recognize that an excessive emphasis on winning can corrupt true learning and academic values. USAYESS believes that both victory and defeat teach valuable life lessons. Athletes are students first and athletes second. The priority of our program is to develop educated young men and women of character who are transformed by their experience in athletic competition and then transfer lessons they learned on the athletic fields to lives of service to others and the country. READ IT CAREFULLY AND BE CERTAIN YOU UNDERSTAND IT BEFORE SIGNING Medical Consent In the event that the Athlete may require emergency medical care, or in the event the Athlete may become ill while participating in any USAYESS (USA Youth Education in Shooting Sports) event, Athlete (and Athlete's parent/legal guardian if Athlete is a minor) hereby gives advanced consent to USAYESS (USA Youth Education Shooting Sports) and Governing Bodies, including their respective volunteers, to provide, through a medical staff of their choice, necessary or advisable medical care and treatment to Athlete. Athlete (and the Athlete's parent/legal guardian if the Athlete is a minor) further agrees to pay any and all medical costs, expenses, and charges to release, waive, discharge, and hold harmless USAYESS (USA Youth Education in Shooting Sports) and Governing Bodies, and each of their respective directors, officers, employees, agents or volunteers, from and against any liability or any claim or demand arising from or connected. ATHLETE SAFETY and CODE of LIVING PLEDGE As an Athlete: I will treat every firearm as if it is loaded at all times. I will keep my firearm pointed in a safe direction at all times. I will not rest my firearm on my foot. I will not rest my hands on the muzzle of my firearm. I will only load my firearm when I am at my shooting station. I will not step off my shooting station or turn around on my shooting station until my firearm is empty. I will carry a semi-auto firearm pointed upwards. I will carry a break-open firearm pointed forward. I know that an orange sticker on a firearm means that it has a release trigger. (USAYESS does not allow release triggers in any event) I realize and understand that safety is everyone’s responsibility. I will learn and obey all rules of firearm safety. Because I am a role model and have the opportunity and responsibility to make a difference in the lives of others, I commit to this Code. I will take responsibility and appropriate actions when I fail to live up to it. As an individual: I will develop my skills to the best of my ability and give my best effort in practice and competition. I will compete within the spirit and letter of the rules of my sport. I will respect the dignity of every human being, and will not be abusive or dehumanizing of another either as a coach, athlete, or fan. As a member of a team: I will place team goals ahead of personal goals. I will be a positive influence on the relationships on the team. I will follow the team rule established by the USAYESS and the adult volunteers. Media Consent and Release As parents/guardian(s) of (athlete), we hereby grant the USA Youth Education in Shooting Sports (USAYESS) Program and their agents the absolute right and permission to use photographic portraits, pictures, digital images, video or audio recordings of my athlete, or in which my athlete may be included in whole or part, or reproductions thereof in color or otherwise for any lawful purpose whatsoever, including but not limited to use in any publication or on any websites, without payment or any other consideration. I hereby authorize USAYESS, and those acting in pursuant to its authority, to use, reproduce, exhibit, or distribute in any medium any media images or recordings for any purpose that USAYESS, and those acting pursuant to its authority, deem appropriate, including promotional or advertising efforts. I hereby waive any right that I may have to inspect and/or approve the finished product or the copy that may be used in connection therewith, wherein my athlete’s likeness appears, or the use to which it may be applied. I hereby release, discharge, and agree to indemnify and hold harmless USAYESS and their agents from all claims, demands, and causes of action that I or my athlete have or may have by reason of this authorization or use of my athlete’s photographic portraits, pictures, digital images or videotapes, including any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of said images or recordings, or in processing tending towards the completion of the finished product, including publication on the internet, in brochures, on banners or billboards, or any other advertisements or promotional materials. I release USAYESS and those acting pursuant to its authority from liability for any violation of any personal or proprietary right I may have in connection with such use. I understand that all such recordings, in whatever medium, shall remain the property of USAYESS at all times. I also understand that no royalty, fee, or other compensation shall become payable to me by reason of such use. I have read and fully understand the terms of this release.
